The papers have to tell the same story
The act, the statute and the application should agree on name, seat, activity and who signs.

This is a StartCG summary, not legal, tax, or investment advice. Confirm fees, forms and deadlines on the official page before you act.
One story, three places
The business portal and the Point of Single Contact describe the same core set for a d.o.o.: a founding act certified as the law requires (a decision for one founder, a contract for several), a statute, proof that the published fees were paid, and the single registration application. If a board of directors is appointed, the portal also asks for the members’ written consent.
Where founders drift
The name in the act and the name on the form. The seat in the statute and the address you later give the bank. The activity you hope to do and the activity you write down. The person who signs the application and the person the statute names as director. Fix the drift before you upload. IRMS is a filing system, not an editor.
Optional annexes
The portal mentions optional annexes for VAT registration and for excise. Attach them only if they apply. A VAT annex is a tax decision. Read the VAT article in this library, then the current rule, before you tick it.
